Clinical Pharmacists

As integral members of our multi-disciplinary team, our Clinical Pharmacists offer a wide array of expertise to support general practice. With their specialisation in pharmacology, they collaborate with GPs and other healthcare professionals to reduce medicine-related workloads, enhance medication accessibility, improve patient safety, and provide guidance on best prescribing practices.

Responsibilities of Our Clinical Pharmacists:

  • Manage day-to-day medication-related issues
  • Consult directly with patients and provide treatment, particularly for long-term conditions such as asthma, COPD, and hypertension
  • Offer expert advice and medication reviews for patients taking multiple prescriptions
  • Provide clinical guidance on treatment options and management

These efforts help ensure optimal medication use and improved overall patient care within our practice.

First Contact Physiotherapists (FCP)

First Contact Physiotherapists (FCPs) are an integral part of the multidisciplinary team (MDT) at The Barkantine Practice; we are experienced clinicians specialising in musculoskeletal (MSK) health,. 

We are able to diagnose, and manage MSK conditions often without the need for a GP appointment.

Our role includes:

  • Leading early MSK assessment and clinical decision-making
  • Supporting self-management with personalised advice and rehabilitation plans
  • Ordering and interpreting investigations when appropriate
  • Referring to other MSK services, including physiotherapy, orthopaedics, and pain clinics
  • Contributing to holistic care planning, including fitness for work and lifestyle advice

Social Prescriber

  • Rebeka Chowdhury

As a Social Prescriber, my role is to connect patients with the appropriate services available in the community according to their nonclinical issues.  I support them by signposting or referring to improve their mental and physical health and wellbeing.

I provide one-to-one appointments over the phone or face-to-face to identify patients’ needs, where they can discuss their concerns and factors that affect their health & wellbeing and get support according to their requirements.

I get referrals from the primary care clinical and nonclinical colleagues, self-referrals as well as from non-GP based staff. E.g. MSK clinic, talking therapy etc.

I work closely with the practice team to ensure that all needs of the patients are met. Particularly to support our patients to tackle with their Social Isolation and Loneliness.
